What to Bring to Your Appointment
Insurance Card
We accept most Insurance.
Photo ID
Parents or legal custodial photo ID.
Medical Records
Previous to your appointment you can authorize for us to request your medical records by signing a Medical Record Release form. Or you can request your child/children medical records from your previous pediatrician and submit to our office before your appointment.
Copays
As required by your Insurance.
